Life certificate

Please note that the Embassy of Belgium is only authorized to sign life certificates for Belgian citizens.

Non-Belgian citizens is Sweden are kindly requested to contact a local Swedish authority (Swedish police, Notarius Publicus, Skatteverket or Försäkringskassan) to sign the form. You can send then your form directly to the Pension services in Belgium.

If you live in Latvia, please contact the Latvian Social Security.

 
For the legalization of a life certificate, the following documents must be submitted:

When visiting the Embassy in person:

  • The form of the pension service, completed and signed by yourself.
  • Your valid Belgian passport or valid Belgian identity card.
  • A recent personbevis (ändamål – utdrag om folbokföringsuppgifter) from Skatteverket. This document must be issued within one week of submission.

By post:

  • The form of the pension service, completed and signed by yourself.

  • A copy of your valid Belgian passport or valid Belgian identity card.
  • A stamped, self-addressed envelope.
